Memorandum of Understanding between OMA Facilitator Training participants and Miami University on behalf of the Scripps Gerontology Center

This level of OMA training provides you with the knowledge and skills to:

1. Plan, conduct, and evaluate OMA sessions for people with dementia in your program.
2. Manage and supervise volunteers in the OMA program.
3. Use OMA Handbook and the right to photocopy handouts from the Handbook for use in your own program.
4. Contribute to OMA online resources.

Scripps OMA Program requirements:

1. One-to-one ratio between elders living with dementia and volunteers.
2. Volunteers are trained using OMA’s volunteer training program.
3. Have a semi/annual OMA art show.

Terms of agreement

By submitting this form, you enter into an agreement with Scripps Gerontology Center at Miami University that you will not exceed the entitlements as stated above. You should note that:

1. Your completion of the OMA Facilitator’s Training does not entitle you to train or instruct others to become an OMA Facilitator.
2. The OMA Handbook is protected under U.S. copyright law. Any photocopying or reproduction beyond the use in your own program is not permitted without a license or a legal claim of fair use under U.S.C. 17 Sections 107-118 or other applicable statutes.
3. You agree not to sell any aspect of the OMA program and to respect the rights of Miami University and Scripps Gerontology Center to safeguard the quality and intellectual property of the program.
4. You agree to work within the person-centered guidelines specified within the OMA Handbook.
5. You agree to keep Scripps Gerontology Center informed of your OMA implementation activities by updating your information annually on the OMA website.
6. You acknowledge that all rights and license to use intellectual property related to the OMA Handbook and program are revocable at the sole discretion of Miami University.

Consent for Use of Video/Photography

By participating in this training, I agree that photographs, video recordings, and/or real time observations made of me and the artwork that results may be used for educational, research, fundraising, and publicity purposes, without compensation paid or due to me for this release and for photos or video recordings that are made.

Consent to be listed as OMA Trained Facilitator

By submitting this form, I agree to have my name and contact information be listed as an OMA Trained Facilitator.
