OMA Pre-Volunteer Experience Survey

The purpose of this research is to compare volunteers’ attitudes toward individuals with dementia before and after participating in the Opening Minds through Art (OMA) program. This pre-survey will take you approximately 5-10 minutes to complete. We appreciate your help with this research.

Although we strongly encourage you to participate in this research, your participation is entirely voluntary. If you do not wish to participate in this research, you can close the survey. 

If you are doing this training as part of a course, your instructor will not know if you participate in this research, and whether you choose to participate or not will have no effect on your grade in this course. Your identity will be removed before we share your responses with your instructors.

Participation in this research consists of you filling out the following questionnaire now and after your OMA in-person experience. Completion of this questionnaire will constitute your consent to participate in the research.
