OMA Post-Volunteer Experience Survey

Please complete the Post-Volunteer Experience Survey and Final Reflection below within two weeks after you have completed your last session working with someone who has dementia in OMA.

The purpose of this research is to compare volunteers’ attitudes toward individuals with dementia before and after participating in the Opening Minds through Art (OMA) program. This post-survey will take you approximately 5-10 minutes to complete. We appreciate your help with this research.

Although we strongly encourage you to participate in this research, your participation is entirely voluntary. If you do not wish to participate in this research, you can close the survey.

If you are doing this training as part of a course, your instructor will not know if you participate in this research, and whether you choose to participate or not will have no effect on your grade in this course. Your identity will be removed before we share your responses with your instructors.

Participation in this research consists of you filling out the following questionnaire before and after your OMA in-person experience. Completion of this questionnaire will constitute your consent to participate in the research.

If your instructor requires it, please submit a one-page final reflection on your entire OMA experience, including the online training. Submit your reflection to your instructors/ coordinators as directed.
